Admissions Manager DescriptionThe Care Center Admissions Manager has responsibility for generating referrals to Community and Care Center and for managing the referral and transfer process. This position is responsible for developing and implementing strategy, work plans, and solicitation tactics to achieve measurable results for the Care Center admissions. This role is responsible for admission decisions for hospital and healthcare referrals. This work entails working on specific campaigns and outreach efforts for the cultivation, solicitation, and securing of referrals and transfer processes.

As the Admissions Manager, you will ensure open and effective communication between the Care Center and referral sources.

Analyze patient needs quickly and match with appropriate community and services to plan for an efficient placement.

As the Admissions Manager, you will bc the initial contact for discharge and transfer of patients from area hospitals and other healthcare settings to Care Center. Participate in the patient transition planning and care process as requested by Care team.

Requirements for the Admissions Manager | Skilled Nursing

  • Deep understanding of Medicare Parts A, B and D, Managed Care / HMO services and guidelines, and the clinical knowledge and judgment to be an admissions decision maker.
  • Ability to establish professional relationships and work collaboratively with individual referral sources, healthcare communities, Care Center staff, and other constituents.
  • Ability to work independently, and to manage remotely.
  • Excellent oral and written communication skills.
  • Computer proficiency in MS Office; working knowledge of e-health records.
  • Ability to travel locally, as necessary, and have valid CA driver’s license with insurance of $100,000/$300,000. Must be able to provide own transportation.
  • College degree in business or related field required. Licensed RN or LVN in the state of CA preferred.
  • Minimum of three years of experience in the Acute Hospital or Long Term Care setting.
  • Track record of successfully obtaining referrals from new and existing sources and meeting assigned referral goals.
  • Knowledge of elder care issues.
